Cow ID

Please identify each animal by freeze brand, ear tag number or other reference. Do not include freshly calved heifers.

Lactation number

Please state the lactation number that the cow is in or has finished.

If the lactation age is unknown, then leave blank. If zero is entered, it will indicate it is a heifer, which it will not allow. Only Cows leaving the herd should be entered.

Last calving date (optional)

Please enter the month and year.

Primary reason

Please select the reason from the drop down box for each cow.

Secondary reason

If there is a secondary or supporting reason for a cow leaving the herd, please select the appropriate code.

Leaving Method

Enter a leaving method from the drop down list. Either Sold for Slaughter, Sold as a Milking Cow, Died on Farm or Casualty on Farm (Shot).
